SCHOOL CHAPERONES
Anytime a person will have minimal contact with children they must complete the clearances required under the Archdiocese’s Safe Environment Policy.
Chaperoning includes but is not limited to class trips, dances, social events, Race for Education, Blue & Gold Day, Home & school board member, hot lunch, etc. These are the steps to complete to chaperone at our school.
Required Clearances
-Pennsylvania State Police Criminal Clearance
-Pennsylvania Child Abuse History Clearance
Required Training
-Mandated Reporting Brochure and acknowledgement form
-FBI Disclosure Statement Form
First, PA State Police Criminal Record Check can be obtained online at https://epatch.pa.gov/home with results available within a few minutes. Please click on the certification form and print a copy of the Response for Criminal Check.
Second, PA Department of Public Welfare Child Abuse Clearance Check is on online at https://www.compass.state.pa.us/cwis/public/home you must make an account, with result emailed to you. Please print a copy of certification when available.
Third, you must sign and return the Mandated Reporter Acknowledgement Form (under documents) after reading the -Mandated Reporting Brochure.
Fourth, you must sign and return the-FBI Disclosure Statement if you have continuously lived in the State of Pennsylvania for ten years (under documents) Otherwise, please call our office.
When you receive the processed clearance papers, please send them to the office for our files.
Criminal, Child Abuse Clearances and FBI Disclosure Statement are valid for five years only. This is free for volunteers.

Thank you for your interest in working in our school. Clearances are required for all situations in which adults have regular contact with children, as required under the Archdiocese’s Safe Environment Policy.

-Mandated Reporting of Suspected Child Abuse Part II (online class)

Classroom Component (Part 1) “Protecting God’s Children” Sign up for 3 hour class.
Online Component (Part 2) Mandated Reporting of Suspected Child Abuse, complete 3 hours online class.
FBI Fingerprint You must register on line before you get fingerprinted. Information is online at https://uenroll.identogo.com/ The service code is1KG6TR after code is entered then click on Manage appointment and follow directions print copy of registration ID# and go to location, Valid Photo ID is required. You will receive two receipts give one to us and we will check results and obtain certificate.
